PLEASE ENSURE THAT YOU COMPLETE THE “APPLICATION FOR LEAVING CERTIFICATE”
FORM IN ORDER TO BE ELIGIBLE TO COLLECT YOUR PROGRESS REPORT. THE FORM IS NOW
AVAILABLE IN ADMINISTRATION.

FILL IN THE FOLLOWING SECTIONS:

A : Fill in ALL.
C : ECA – Find Muhammad Sia Abdullah @ GF.62
D : SWO – Find Siti Mazulianawati Haji Majid @ 1A.25
E : CHIEF LIBRARIAN – Find Hjh Pusparaini Hj Thani @ Library
F : COMPUTER MANAGER – Find Jennifer Voon Nyuk Hiong @ 2A.39
B : HOD – Find Narayanan N Jr @ 2F.06

Fill in only – ‘Examinations taken with results’ – fill in all units since Stage 1 until Stage 3.

E.g. Stage 1 : PO – M, CO – M, BE – M etc

Stage 3 : BPC – M, OBE – M, BIS2 – M

‘Comments on Student’s Attendance and conduct/attitude’ will be given if all
information in Section C,D,E and F are completed. *HOD will be the last to fill in the
form*

ONCE ALL SECTIONS ARE COMPLETED AND THE FORM IS SUBMITTED TO THE HOD, YOU
CAN THEN COLLECT YOUR PROGRESS REPORT.

THE PROGRESS REPORT SHOULD BE AVAILABLE MONDAY, 5 JANUARY 2009 @ AHOD’S OFFICE - 2F.101.

CONGRATULATIONS to the students who have passed! ;D

To the resit students, NEVER GIVE UP! Try your VERY BEST this last chance and do revision early (now)! Dont wait until you "feel like studying". Just do it! Throw away other stuffs & get those papers & mindmap it, use keywords, flashcards (carry everywhere you go), etc. Understand first, then memorize where neccessary. Test your memory yg recalling few times. Get enough sleep and eat well few days before the resit exam soon. Study Smart & Good Luck ok! We are all here to support you.

To repeat students, NEVER GIVE UP too! Whats done is done. Start fresh now & look/ prepare for your future. Its gona be easy the second time around. This is also your very final chance. Ignore the negativity and don't care abt what others say abt u being a repeater and loser or watsoever. Some of us have been there before. Show and prove them you can get distinction and merits! Surprise everyone that you can be better than us too ok!

Finally, I wish you all the best of luck in future. Stay strong & start fresh!
